Getting SMTP error while using PHpmailer to send mail through my website

I am trying to send email from my website through phpmailer class in php
The code is given below but am trying to send the email i am getting this two errors .

SMTP Error: Could not connect to SMTP host. Message could not be sent.

Mailer Error: SMTP Error: Could not connect to SMTP host.

What going wrong .please Help ?
